Tuition, Books, and Fee Payment
Tuition, books, and fee payments are the responsibility of each individual student. If an agency, such as JTPA is funding a student, the student is responsible for obtaining the necessary forms from the funding agency and submitting them to the UBATC financial aide officer in adequate time to ensure payment. A student will not be allowed to continue in class if tuition has not been paid.
Financial Aid and Scholarships
Financial aid may be available to UBATC students and is awarded on the basis of need and availability. The student may obtain information about this resource through the financial aid officer of the Uintah Basin Applied Technology Center. Application for financial aid must be made well in advance of the scheduled course.
Clinical/Lab Requirements
Each student must be covered by malpractice insurance through out the year. UBATC has a blanket policy covering each student. The premium for this insurance is paid each quarter from part of your UBATC fees. However, students are highly encouraged to maintain their own malpractice insurance policy. If you have a question on how to obtain malpractice insurance please see your faculty advisor.
The Uintah Basin Applied Technology Center does not accept responsibility for injury or illness that occur while students are enrolled in the practical nursing program. Students are strongly encouraged to carry health insurance. Each student must have an up to date immunization record. This means a DT within the last ten years, rubella and rubeola since 1973, or a recent titer proving immunity. Students must also have the results of a tuberculin test within the past six months and be current throughout the year. If the skin test was positive, a chest x-ray is needed.
Students will be required to adhere to state recommended universal precautions to protect themselves and to prevent the spread of disease in the clinical areas. Hepatitis B vaccination is required to participate in all clinical areas. It is the responsibility of each student to provide proof (either by public health vaccination record or by signature of health care provider administering vaccinations) to the nursing department before entering any clinical facility in a student capacity. A copy will be placed in your permanent file.
In the event that a student has an exposure such as a needle stick, it is their responsibility to follow the exposure protocol set up by the health care institution that they are doing their clinical at. This also means that the student, not the institution or the school is responsible for the cost related to the exposure injury.
Student Records
A cumulative record is kept for each student, beginning with application to the nursing program. Records of attendance, grades, personal references and other pertinent information, such as experience booklets, will be added to the file. This information is confidential, and will be released only on written permission of the student. The record is kept on file after graduation for three years.
Student Health
As a student nurse, you are a health care provider, and as such we encourage you to exert every effort to maintain optimal health. Your example will influence others far more than any other teaching you can do. Illness may cause absence that will affect your grade.
We encourage you to be open and honest about any health problems, which may exist. Problems that exist when you enter the program, such as diabetes, epilepsy, heart conditions, drug related problems, emotional problems, or any other problems which might require attention or special considerations must be documented when you enter the program. Failure to disclose a known serious health problem could result in dismissal from the program. This regulation is made out of concern for your protection, so that we might be prepared to deal with a health crisis.
Student Honor Code
The Honor Code is established to promote professional conduct and personal integrity on the part of all nursing students. Each student in the Practical Nursing Program is expected to abide by the following sanctions. A student may be placed on probation, suspended or dismissed if in violation.
- Copying from another student’s test paper.
- Using material during a test not authorized by the person administering the test.
- Collaborating with any other person during a test.
- Knowingly obtaining, using, buying, selling, transporting or soliciting in whole or part the contents of an unadministered test.
- “Plagiarism” means the appropriation of any other person’s work and the unacknowledged incorporation of that work in one’s own work offered for credit. This includes purchased or borrowed papers.
- “Collusion” is the unauthorized collaboration with another person in preparing work offered for credit.
- Furnishing false information to faculty with the intent to deceive.
- Forgery, alteration or misuse of UBATC documents or records.
- Theft or malicious destruction, damage or misuse of UBATC property or the private property of another, whether occurring on or off campus.
- Possession, use or distribution on campus and/or clinical settings of any alcohol, narcotic, or dangerous or unlawful drug or controlled substance as defined by the laws of the United States or the State of Utah except as expressly permitted by law.
Grooming
Uniform
a: Tailored white dress or pantsuit. (Ruffles, lace, permanently wrinkled material, “see through” material and scrub uniforms are not acceptable.)
b: *Uniform will be professional, clean, wrinkle free, proper fit and appropriate length.
c: Student patch is required and must be secured to upper area of left sleeve and it should lay flat on uniform and lab coat. Patch needs to remain in good condition.
d: Dress length must be below the knee. White hose or white sox. Clean, well fitting white leather shoes. Plain white sweater or lab jacket, with patch secured on upper area of left sleeve, may be worn with uniform when necessary.
Name badges
a: Must be worn at all times in the clinical areas.
b: No defacing of name badge permitted. (No stickers, pins, etc.)
c: Name badges are the property of UBATC, and will be turned in at the completion of the program.
d: Replacement cost for lost, stolen or destroyed badges is $5.00 and still remain the property of UBATC PN Program.
Immaculate grooming
a: Daily personal hygiene including deodorant is essential because of the close proximity to clients and others.
b: Since odors of any kind may be offensive to clients, products with strong odors should be avoided while working in the clinical setting (i.e., perfumes, colognes, after shave, tobacco, etc.).
c: Hair must be contained, clean, and worn off the face and in a conservative style to provide protection for the client.
d: Facial hair must be clean, trimmed and worn in such a manner that will not obstruct nursing activities.
e: Simple make-up and clean short nails are expected in the clinical setting. Clear or light colored nail polish may be worn. Artificial fingernails are prohibited.
f: For the safety of the client, and student, jewelry (with the exception of a wedding ring and/or wrist watch) is not to be worn in the clinical area.
g: For pierced ears, small simple posts may be worn in the ear lobe, in the clinical area. Multiple earrings are not allowed in the clinical area. Visual body fashion jewelry is prohibited.
h:Gum is not to be chewed in the clinical area.
Additional criteria may be defined by nursing instructors in specified clinical areas. Failure to meet any of the above criteria may result in the student being excused from the clinical area.
If differences exist between the nursing program and agency policies, the nursing program will also conform to the agency policies.
